Our Company:Adoration Home Health and HospiceOverview:Private Duty LPN G-Tube CareOverview:We are seeking dedicated LPNs to provide one-on-one in-home care with a split schedule between Jackson, TN, and Hornbeak, TN. This role offers flexibility and variety, as youll work in Jackson during the first half of the month and Hornbeak during the second half.Location & Schedule:Jackson, TN: 1st through the 15th of each monthWeek 1: Monday, Tuesday, Thursday, Friday (7:30am5:30pm)Week 2: Monday, Tuesday, Saturday, Sunday (7:30am5:30pm)Hornbeak, TN: 15th through the 30th of each monthSame schedule split appliesHighlights:Work across two locations while providing consistent, personalized careFocused, one-on-one patient care in home settingsComprehensive training and supportive teamBenefits:Flexible schedulingCompetitive weekly payDailyPay optionTuition reimbursementProfessional growth opportunitiesQualifications:Must have an active LPN licenseG-Tube care experience requiredCPR certificationResponsibilities:Perform selected nursing procedures within the scope of LPN practiceProvide patient and family teaching according to plan outlined by the RNMonitor patient's vital signs and make specific measurements and observationsMonitor and record intake and output. Report significant changes immediatelyObserve, assess and record patient's physical and mental condition and reports any changesAdminister medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordinglyAttend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assignedQualifications:Licensed Practical Nurse / LPN license in good standingPrevious private duty, home health, hospice, hospital or skilled nursing experience preferredActive CPR certificationFamiliarity with electronic health recordsStrong problem solving and critical thinking skillsReliable transportation, valid driver's license and current automobile insuranceAbout our Line of Business: Adoration Home Health and Hospice, an affiliate of BrightSpring Health Services, provides quality and compassionate services in the comfort of home, providing support for patients, families, and caregivers in their time of need.
