As a member of our team, your compassionate care, communication skills, and understanding of the psychosocial dynamics of illness, loss, and death will be instrumental in providing direct patient care during the final stages of life.Key Responsibilities:Assess the psychosocial status of patients and their families/caregivers related to the patients terminal illness and environment.Communicates any special needs to the interdisciplinary group.Evaluate family dynamics, caregiver abilities, volunteer and bereavement needs, etc., and provide interventions as necessary.Provides information to patients and their families/caregivers and community agencies.Key Qualifications:LMSW required. Meets state-specific guidelines, education, and licensure requirements. Meets personnel qualifications stated in 418.114(b)(3).Minimum of one (1) year of experience in health care; hospice experience preferred.Understands hospice philosophy and issues of death/dying.Demonstrates good verbal and written communication and organization skills.Possesses and maintains current CPR Certification.Must be licensed with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.Your Role and Growth OpportunitiesUpon joining the Legacy family, we provide a comprehensive training program to equip you with the necessary knowledge and skills to excel in your role.
