Job Description Are you a family medicine physician interested

may be the ideal place for you! Job Summary The Family Medicine
Physician is responsible for providing medical care to people of
all ages; responsible for treating, testing, diagnosing, and
monitoring diseases and providing general care to community
members. Responsibilities Provide compassionate, appropriate and
effective clinical care for patients of all ages. 100% outpatient
care if not providing OB services. Maintain good quality medical
records. Work within a team collaborative model at a Patient
Centered Medical Home. Advocate for quality care on behalf of
patients and the organization. Provide care and treatment.
Diagnose, treat, and monitor diseases, disorders, and conditions.
Prescribe appropriate medications to treat patients in accordance
with standardized procedures and company policy. Perform pap
smears, pelvic and breast exams. Conduct mood and emotional
well-being assessments during post-partum appointment. Collaborate
and consult with other medical and nonmedical personnel.
Effectively monitor patients' progress throughout treatment plan
and care, and provide continuous improvement on patient outcomes.
Educate patients on the importance of treatment plans and
communicate side effects and risks of medications prescribed.
Educate patients on disease prevention and health. Demonstrate
effective and detailed written documentation skills. Conduct
routine check-up appointments and maintain follow-up throughout
Demonstrate excellent customer service and communication with
patients. Able to multitask and manage assigned projects and tasks
as directed. All other duties and responsibilities as assigned.
Education, Training, and Experience Required MD/DO degree.
Successful completion of family medicine residency. Licensed in the
state of New York and/or Pennsylvania or willingness to obtain
licensure. Board Certified or Board Eligible. DEA Certification
required. Ability to demonstrate excellent independent judgment,
problem-solving, analytical, and collaborative skills in order to
organize and prioritize multiple tasks, providing patient-centered
leadership and direction. Must have the emotional intelligence that
may include, but not limited to, patience, sensitivity, and the
compassion to work with patients that may be stressed, anxious, and
difficult. Ability to work as a team. Must have strong computer
skills. Must demonstrate detailed, effective writing/documentation
